Support after discharge

After hospitalisation, surgery, or during recovery, some patients may need additional support at home. Sevagan helps families identify the type of home-care or nursing support they need and assists in arranging a suitable service provider.

How we help

  • Understand the patient’s home-care or nursing-care requirement
  • Help identify a suitable home healthcare provider
  • Assist with arranging nursing or home-care services
  • Coordinate the service based on the patient’s requirements and preferred schedule
  • Help the family communicate their requirements to the care provider
  • Provide coordination support if requirements change

When you may need home-care support

  • Nursing care after surgery or hospitalisation
  • Assistance for elderly or dependent patients
  • Patients who need help with daily activities at home
  • Post-operative or recovery-period support
  • Patients whose family members cannot provide full-time assistance
  • Families living in another city or abroad who need local support

Before arranging care

It is useful to know:

  • What type of assistance does the patient need?
  • Does the patient require a trained nurse or a caregiver?
  • How many hours of support are required?
  • When should the service begin?
  • Where is the care required?

What to tell us

To help us arrange the right service, please share:

  • Patient’s age
  • Current health condition and care requirements
  • Doctor’s advice, if applicable
  • Expected discharge date, if applicable
